VOC Emissions Testing
Chain-of-Custody

Complete and sign this Chain-of-Custody form when you submit a product sample for VOC emission testing. Use one form for each unique sample. The form asks for:

• Customer and manufacturer information
• Product sample name and identification number
• Manufacturing, collection, and shipping dates for product sample
• Requested chamber test protocol
• Modelling or data analysis protocol
• Intended application of test data

Phthalates in Toys & Products Plus Other SVOC Testing
Chain-of-Custody

Complete and sign this Chain-of-Custody form when you submit product & material samples, such as toys, for analysis of phthalate esters & other SVOCs. Multiple samples can be submitted on one form. The form asks for:

• Customer and manufacturer information
• Product sample names and identification numbers
• Manufacturing date(s), if known
• Requested analysis type, such as phthalate esters by CPSC method
• Requested turnaround time (extra charges apply for 24 or 48 hour turnaround)

IAQ VOC Services
Chain-of-Custody

Use your company’s form, or complete and sign this BkA Chain-of-Custody form when you submit field-collected air samples for analysis. Our form:

• Identifies your project or job
• Indicates whether you have received air sampler(s), sampling pumps and other materials from BkA
• Identifies samples by tube number and/or sample ID(s)
• Provides sample collection data if reporting of concentrations is desired
• Requested standard(s) for analysis, such as formaldehyde by ASTM D 5197
